Serving the Special Dietary Needs of Students:
Students with Special Dietary Needs who require changes to the basic meal are required to provide documentation with accompanying instructions from state-licensed healthcare professionals and registered dietitians (RDs) and Registered Dietitian Nutritionists (RDNs).
Please have your child's healthcare provider complete and sign the below form (Food Allergy/Disability Diet Modification form). Turn the form into the Nurse at your child's school. The Nurse will provide the completed form to the Child Nutrition Department.
